Calibration Physical Therapy LLC is committed to providing high-quality care in a professional and transparent manner. This Financial Policy outlines our expectations regarding payment, insurance, and financial responsibility.

We believe clear communication about billing helps prevent misunderstandings and allows you to focus fully on your recovery.


1. Insurance and Coverage

Calibration Physical Therapy may accept assignment of benefits from certain insurance carriers. However, insurance coverage is an agreement between you and your insurance provider.

As a patient, you are responsible for:

  • Verifying your benefits and coverage prior to treatment
  • Understanding your deductible, copay, coinsurance, and visit limits
  • Any services deemed non-covered by your insurance plan

While we may assist with benefit verification as a courtesy, verification does not guarantee payment by your insurer.

If your insurance company denies payment for any reason, the balance becomes the patient’s responsibility.

2. Copays, Coinsurance, and Deductibles

Copays, coinsurance amounts, and unmet deductibles are due at the time of service unless other arrangements have been made in advance.

Collection of copays is required under many insurance contracts and federal regulations. Failure to collect copays may constitute insurance fraud.

If your deductible has not been met, you may be responsible for the full contracted rate until your deductible is satisfied.

3. Self-Pay Services

Patients without insurance coverage, or those choosing not to use insurance, are responsible for payment at the time of service.

We may offer transparent self-pay rates and, in certain cases, payment arrangements. Payment plans must be agreed upon in writing prior to treatment.

4. Appointment-Related Fees

Late cancellations and missed appointments may incur fees as outlined in our Cancellation Policy.

These fees are not typically billed to insurance and are the patient’s responsibility.

5. Outstanding Balances

Statements may be issued for unpaid balances.

If payment is not received within a reasonable timeframe:

  • Reminder notices may be sent
  • Future appointments may be postponed
  • Accounts may be referred to a third-party collection agency as permitted by law

We will make reasonable efforts to work with patients experiencing financial hardship.

6. Returned or Failed Payments

Returned checks, declined credit cards, or failed electronic payments may incur additional processing fees.

7. Financial Hardship

If you are experiencing financial hardship, we encourage you to contact our office to discuss possible options. We are committed to working in good faith with our patients whenever possible.
