THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Calibration Physical Therapy LLC (“Calibration PT,” “we,” “our,” or “us”) is required by law to maintain the privacy of your protected health information (PHI) and to provide you with this Notice of Privacy Practices explaining our legal duties and privacy practices.
We are required to follow the terms of this notice currently in effect.
1. How We May Use and Disclose Your Health Information

We may use and share your health information to provide, coordinate, or manage your healthcare treatment. This may include communication with physicians, specialists, or other healthcare providers involved in your care.

We may use and disclose your health information to obtain payment for services we provide. This may include billing your insurance company, verifying coverage, or collecting outstanding balances.
C. Healthcare Operations
We may use and disclose your health information for operational purposes, including:

- Staff training and credentialing
- Compliance and auditing activities

A. Right to Inspect and Obtain Copies

If you believe information in your record is incorrect or incomplete, you may request that we amend it.
C. Right to an Accounting of Disclosures
You may request a list of certain disclosures of your health information made by us.
D. Right to Request Restrictions
You may request restrictions on how we use or disclose your health information. While we are not required to agree to all requests, we will comply when required by law.
E. Right to Request Confidential Communications
You may request that we contact you in a specific way (for example, by phone at a certain number or by mail to a different address).
F. Right to a Paper Copy of This Notice
You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
5. Our Responsibilities
Calibration Physical Therapy is required to:
- Maintain the privacy and security of your protected health information
- Provide you with this Notice of our legal duties and privacy practices
- Notify you in the event of a breach of unsecured protected health information
- Follow the terms of this Notice currently in effect
We reserve the right to change this Notice. Any revised Notice will apply to all protected health information we maintain and will be posted in our office and on our website.
