Your Right to a Good Faith Estimate
Under federal law, you have the right to receive a “Good Faith Estimate” explaining how much your medical care may cost if you are uninsured or choosing not to use insurance.
Calibration Physical Therapy LLC complies with the No Surprises Act and related federal regulations.
Who Is Eligible?
You are entitled to receive a Good Faith Estimate if you:
- Do not have insurance, or
- Choose not to use your insurance for physical therapy services
What Is a Good Faith Estimate?
A Good Faith Estimate is a written explanation of expected charges for medical services, including:
- Evaluation fees
- Treatment sessions
- Other reasonably expected services related to your care
The estimate is based on information known at the time it is created. Actual charges may vary depending on your individual treatment plan.
When Will You Receive It?
If you schedule services at least 3 business days in advance, you have the right to receive a Good Faith Estimate in writing before your appointment.
You may also request a Good Faith Estimate before scheduling services.
What If the Final Bill Is Higher?
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the charges through a federal patient-provider dispute resolution process.
